Guardian Consent Form (Minors)

1. Scope
This form applies to participants under 18. A parent or legal guardian must read, sign, and upload the signed PDF before registration approval.
2. Guardian declarations
I confirm that:
- I am the lawful guardian of the minor participant and authorised to consent.
- I have read and agree to the Terms of Service, Privacy Policy, and Copyright & Licensing notice.
- I understand the process includes AI interview registration, uploads, possible public display, and jury review.
- The minor will follow on-site rules and staff instructions during the event.
- 「HKAIIFF city recommendation seats」 are evaluation labels only — not monetary payouts or equity arrangement offers.
3. Data and imagery
I consent to processing of the minor’s name (may be masked in public views), contact details, works, and necessary guardian contact data for event operations.
4. How to sign
- Download the PDF template (or follow city-site instructions).
- Sign by hand with date.
- Upload via the registration flow (pending review).
5. Withdrawal
Email hack@hkaiiff.org to withdraw consent. Lawfully published archive content may be retained per policy.
